Chapter 2 - THE SCALPEL OF TRUTH

The clinical hum of the cardiac monitor suddenly spiked into a rapid, frantic rhythm as the ultrasound probe glided across Claire’s cold, gel-slicked abdomen.
I kept my eyes locked on the monitor, refusing to acknowledge the two ghosts standing frozen in the doorway of Trauma Bay 4.
"Free fluid in the hepatorenal space," I said, my voice cutting through the thick, suffocating tension like a clinical blade.
"The ultrasound shows a massive pocket of internal bleeding, and the gestational sac is ruptured in the right fallopian tube."
"Activate the massive transfusion protocol, prepare two units of O-negative blood on the rapid warmer, and get the surgical consent forms ready."
My mother, Patricia, took a staggering, unstable step forward, her manicured hand clutching the lapel of my father’s coat.
"Emily, please... she’s your sister," she pleaded, her voice cracking with a high-society panic that I had not heard in five long years.
"You can't let her die... you have to do something!"
"I am currently doing my job, Patricia," I said, my voice completely devoid of any family warmth.
"But in this room, she is not my sister."
"She is a patient with a life-threatening obstetric emergency, and if you do not step back behind the red line, security will escort you out."
My father, Richard, stared at me, his eyes wide with a profound, paralyzed shock as he watched his discarded daughter direct the medical team.
He saw the absolute, unquestioned authority in my eyes, the way the nurses and residents moved immediately to execute my commands.
The man who had once cut off my rent before midnight was now looking at me as if I held the very keys to his existence.
"Emily..." he whispered, his hands trembling as he reached out.
"We didn't know... we were told you dropped out."
